Decision making in challenging endodontic cases
Every dental practitioner's optimal goal is to restore function and esthetics within a healthy periodontium. Clinicians often face hard challenges regarding decision-making in treating massively compromised teeth.
Reviewing the literature reveals that orthodontic extrusion, Intentional replantation, and auto-transplantation could be valid options whenever the involved teeth are beyond treatment by conventional means. Also, the obturation of oozing canals provides another challenge, which may require multiple appointments; however, the recent suggestion of using MTA as a permanent obturating material has proven to help overcome such a problem.
Lastly, adopting revascularization procedures as a treatment in nonvital teeth with open apices should not be excluded as a valid option.
The review of the current literature supports the reasonable follow-up success of the five cases included in the presentation.
